Services & Programs

Services available at this Canadian Mental Health Association Lambton-Kent location include:

  • Case Management: This position is responsible for providing individualized and highly flexible support to people who have a serious mental illness utilizing a biopsychosocial rehabilitative approach to service. The Case Manager will be required to develop, implement and evaluate individualized plans of care with clients on an ongoing basis. Additionally, the position is responsible for the facilitation of psychoeducational groups as required, court support (Gladue), and brief psychotherapy.

How to Access 

Self-referrals or by family, physicians, hospitals or community organizations are received through the Sarnia Lambton Mental Health Access Committee. Contact CMHA for assistance in making referral.

Access committee meets weekly to review referrals. Services are for individuals that have a severe and persistent mental illness causing disability. Contact Intake for more information.
